FDChart output as a COBOL FD

This is a sample of FDChart output as a COBOL FD.  Starting positions of each field are in columns 1-6, field length in bytes is in columns 73-80.  FDChart will indent each level by four spaces if there are less than six levels in the FD; otherwise it will indent each level by two spaces.
